You can retrieve LOGIN, PASSWORD or EMAIL information independently if you know two of those three pieces information.

1. Retrieve LOGIN information: use the top section form only. You will need password, email, birth year, zip code and correct Captcha to retrieve your login name. If you had selected not to save your login name (this is the default option that PHRsT only saved your login name in its encryption form) when creating your account, you will be given a temperate login name. Please use the Update Form to change this temperate login name to a more meaningful login name.
2. Retrieve PASSWORD: use the middle section form only. You will need login name, email, birth year, zip code and correct Captcha to get a temperate password. Please use the Update Form to change this temperate password to a more meaningful password.
3. Retrieve EMAIL: use the bottom section form only. You will need login name, password, birth year, zip code and correct Captcha to retrieve your email if it had been saved before.
